UMC-Northport Again Recognized For Patient-Centered Care

University Medical Center’s Northport location was recertified as a Patient-Centered Medical Home in May in recognition of its commitment to place patients at the forefront of care, and to advance quality in the health care it provides.

The recertification was provided by the National Committee for Quality Assurance. The NCQA is a national organization that works to improve the quality of U.S health care.

UMC’s Family Medicine Clinic in Tuscaloosa has also been recertified as a PCMH. The Pediatric Clinic at UMC in Tuscaloosa received certification as a PCMH in early 2021.

NCQA recognition means medical practices are committed to providing care that is patient-centered, accessible, continuous, comprehensive and coordinated, and that focuses on quality. The PCMH model uses a care delivery team, led by a primary care physician, that delivers coordinated and integrated care and is proactive in providing preventive, wellness and chronic illness care – all with the patient at the center of the health care experience.

Research shows that the PCMH model builds better relationships between patients and their clinical care teams, and helps patients be more compliant and successfully manage chronic health conditions.

UMC is the largest community medical practice in West Alabama, with locations in Tuscaloosa, Northport, Demopolis and Fayette, and nearly 160,000 patient visits in 2020. UMC provides primary care-focused health services in family medicine, internal medicine, pediatrics, women’s health, psychiatry, geriatrics, neurology and sports medicine.
